The answer is usually, one side of the tongue is affected, and when the person sticks out his or her tongue, it turns or points toward the side that is damaged. Damage to the hypoglossal nerve causes paralysis of the tongue.  The nerve originates in the brain in the medulla oblongata and attaches to the underside of the tongue.
