Which connective tissue surrounds each fasciculus?
endomysium
epimysium
perimysium
aponeurosis
tendon

The ones that surroundes each fasciculus would be the perimysium. Muscles was one of the hardest bits when I took A&P which I had to 2 years of for my degree plan. I learned the connective tissues by going over each layer. I normally started on the inside:
Sarcolemma is the cell membrane that encloses the muscle cell, next you have the endomysium which wraps individual muscle fibers. Perimysium wraps each bundle of muscle fibers also called fasicles then the epimysium that wraps the whole muscle then fascia "deep" that cover the epimysium.
