prevent any errors, he decided to use the critical accident method as a performance evaluation system that

performance based on the number of occurred errors by each doctor or nurse annually and will determine
their incentives and rewards accordingly. The commitment of all members of the department to this system
has reduced the errors of nurses and doctors by 50% annually.
