Purpose: CSW and the consumer met at the community in SE for the CSW to assist her with developing healthy coping skills in order to decrease negative thoughts, self doubt and triggers. The CSW also educated her on how to improve her self worth to avoid self doubts as well as boost her self-esteem.

Intervention:

CSW and the consumer discussed and CSW educated the consumer on how to improve her feelings towards herself and her perception of herself, how to build better self confidence and also improve her feeling and thoughts related to doubts in order to return to her previous level of effective and positive daily life functioning. CSW reviewed the consumers ability to individually perform tasks and avoiding negativity through performing self-positive reassurances, continuously avoiding negative thoughts. The CSW also mentioned to him some coping skills like exercising often, engaging in leisure and fun filled activities and taking part or watching positive debates to better understand how others deal with their negative thoughts problems.

CSW urged MS Thomas to improve her efforts to decrease feelings related to negative thoughts by upholding positive self-esteem and views about herself to help her progress and prevent a relapse. CSW recommended her to endeavor in the direction of maintaining favorable and dynamic social relationships that will facilitate the lessening of depressive moods daily through creative and positive thinking and self-talk as well.

CSW guided the consumer in developing and initiating an increased awareness of triggers as well as the coping strategies and skills needed to effectively deal with his triggers to her negative thoughts feelings.

Response:

The consumer reported the frequency at which she always feel down about herself. She said she is always having negative thoughts and low self esteem. Consumer said she utilizes coping skills like taking a walk and spending time talking with relatives/friends. The consumer also said she has been applying some of the coping skills learnt in the past but she has not noticed any significant change. She appreciated CSW for the time and efforts in motivating and encouraging her. Mr Thomas promised to continue following up with her treatment plan guidelines and objectives as the session came to an end.

Plan:

CSW will continue to work with Ms Thomas on dealing with her ways to avoid triggers and reduce feelings of negative thoughts.
