Ahhh, art.
the only real way to discover the mind of a dark, brooding, tourtued hearted artist. the picture itself, will tell you a milion stories.
finding the right story  however, that's what is up to the person who is looking at the detailed emotion in front of them. 

luckily, i can with this. 
(fact that this was van goghs nurse/doctor! keep that in mind)
firstly, look at the color scheme. 
they seem a dull. the blacks are faded- who knew you could paint black in a faded way? - the blend of whites, greens and blues, only bringing the tone to a cooler appeal, letting us know that this is a subject of if not great horror/pain that is lurking his thoughts, but a possible sadness/loss and/or confusion. 

the pale yellow of his skin(which is also rather dull for such a bright color) tells what i see myself, of a man who is alive, possibly living a good life too, but also a man who has seen much in his lifetime. if you look closer at the greenish hue around his eyes, he looks rather tired too. this could hint at a lack of sleep, stress, or sickness. 

but before we get too far, notice his attire. his hat looks law/armed/medically forces designed, am i correct? look at his coat. think back to when you have seen a movie/films based on over-seas war or medical debutes. i think what i am seeing is a captain/doctor's coat. so now, we have an old "war/doc. hero," if you , in our mental files. 

this would explain the persona of sadness/loss/pain/tiredness/confuson etc. 

beside him, on the table that is- once again faded -orange with dim green smudges, there are what look to be two golden books. this can either mean that this man is intellegent, that he likes to read, or that they could be his religion if he has one.

for what i can see, the best way to caption this portrait, would be something like:

"the man who has known life." 

this points out that he has lived, is smart, and had a knickknack for /savng lives. i hoped this ! if you need a longer caption, let me know.
