PROCEDURE 21-1: REPORT CONFIRMED OR SUSPECTED DISEASE, MICROORGANISM, INFECTION OR CONDITION TO THE APPROPRIATE AGENCY
Task: Reporting of cases of communicable disease is important in the evaluation and planning of disease control and prevention programs, to assure appropriate medical therapy, and in the detection of common-source outbreaks. (Note: This procedure will vary slightly depending on state laws.)
Conditions: • List of reportable diseases
• Confirmed laboratory report or request of the provider for suspicion of reportable disease, infection, microorganism, or condition
• Reporting phone number, fax number, or Web site
• Case report from specific to reportable conditions
Standards: Perform the Task within 10 minutes with a minimum score of 70 percent. Scoring: Determine student's score by dividing points awarded by total points possible and multiplying results by 100.
Competency Assessment Information
The following is a list of possible topics to use as scenarios for reporting of cases of communicable disease (refer to Table 21-5 in the textbook for addition diseases that must be reported). Document the report in the patient's medical record:
• AIDS

• Lyme disease
• Measles
• Rabies
• Rubella
• Syphilis
• Tuberculosis
