How would you alert the document author if you found a critical error or discrepancy in a dictation?

I would simply phone or e-mail them or tell them directly about the error and say that according to my understanding (use the "I" statement) there appears to be a such and such error that needs to be corrected so that the text does not say give the wrong impression.
