How do you write y-x=5 in slope intercept form

Slope intercept form is y = mx + b

Isolate the y. Add x to both sides

Y - X (+X) = 5 (+X)

Y = 5 (+X)

Note however, that 5 is the b (constant), and so it needs to behind 1(x) (the slope and variable.)

Y = X + 5 is your answer
