First things first, you have to know your positives, negatives, zeros, and undefined.

To make an equation from the graph you have to know your formula.

y=mx+b

You do the rise first. The rise on the graph is 3. The run is 2. So, that'll make it 3/2. m=slope.

b=y-axis. b=1.

i believe it is the square root of 122 because when you turn it on it's side, to where the 5 inches is on the bottom, it looks like it is the same length.
