Well there is lots of ways that poor health can affect your social life. 
1. if you don't feel well you wont want to go out and socialise 
2. if your in poor health when you do go out your friends might pick an activity that you cant do because your not in prime condition 
3. also when you do go out to be social you my not enjoy it as much as you would if you felt better.
