I'm pretty sure number 4 is H=4w+36. Keep in mind that the flowers are growing at a rate of 4 inches each week so multiplying 4 by w is needed in the equation. I'm not sure about 5, sorry

#4 is 4w+36 because 4 is the amount it grows per week while variable X means the amount of weeks that passes, 36 is the initial growth rate;its 4w+36 because it state the plant grows.

#5 is the initial golf balls increased from 3500 to 4000, since its a constant it doesn't change the rest of the equation
