Anyone out there who could help me out with the diagnosis? My file had this labelled as IHD, but I'm inclining towards a stroke? Treatments employed for this patient were BB, CCB, ACEI, clopidogrel and statins which isn't usually for a stroke? Is this a Drug therapy problem then? I'm just very confused. Case summary (chief complaint):
Two falls overnight associated with confusion and left sided weakness. Chest pain also since last fall at 4am.
HPI: patient presents with left, upper extremity & lower extremity weakness, facial droop, altered speech and altered gait. The onset was 5 hours ago. The course/duration of symptoms is constant. Location: left face; upper extremity; lower extremity. Character: weakness, difficult to speak and off-balance. The degree at onset was minimal, at present - moderate.
Risk factors: DM, hyperlipidemia, HTN and obesity.
History of left sided chest pain before weakness associated with sweating without radiation lasting for few minutes.
